The medical drama "Brain" involves three doctors at Chunha University Hospital.
Lee Kang-Hoon (Shin Ha-Kyun) is an excellent but arrogant brain surgeon at Chunha University Hospital. He comes from a poor family and is still haunted by the painful memories of his father's death due to being rejected by the doctor. Kang-Hoon has a strong desire to succeed and proves that he is the best. His rival is Joon-Suk (Jo Dong-Hyuk), who comes from a family of doctors and has everything basically. Kim Sang-Chul (Jung Jin-Young) is known to be the best brain surgeon at Chunha University Hospital. He also cares for his patients and hates Kang-Hoon very much.
One day, a young man collapsed in an indoor swimming pool and was taken to the hospital. Patient fainted upon arrival. Kang-Hoon orders to prepare for surgery, but Joon-Suk asks them to call Kim Sang-Chul for surgery. Kang-Hoon replied that he didn't have enough time and perform the surgery on his own, while the other doctors observed the surgery.
During the operation, there was a dangerous moment when a patient's bulge was ruptured. At this time, Joon-Suk calls Kim Sang-Chul. Kim Sang-Chul ran to the operating room. Kang-Hoon WLife managed the operation well and the surgery was a success, but he left the last clip to the hospital deputy director. Kang-Hoon wants to have the favor of the deputy director. Because of her arrogant attitude, he was not welcomed by anyone at the hospital.
While Kang-Hoon is resting in his office, he gets a call from his sister. Kang-Hoon said he was busy and hung up. He attends a conference of facility doctors. There Kang-Hoon questioned the subordinate doctors, but none of them could answer his question. Kang-Hoon then asks their chief doctor, Ji-Hye (Choi Jung-Won). Ji-Hye is not there. Kang-Hoon yells at them to follow him.
On the way, Kang-Hoon and the group of friends happen to meet Kim Sang-Chul and Ji-Hye. Kim Sang-Chul blamed Kang-Hoon for not checking his patients after the surgery.
When Kang-Hoon returned to his office, he started to blame others and especially Ji-Hye, who didn't graduate from Chunha University. Ji-Hye talks again with Kang-Hoon. Kang-Hoon decides to take revenge by working. While Ji-Hye was writing a report requested by Kang-Hoon, Joon-Sook surprised her. Ji-Hye replies that she can't understand why Kang-Hoon hates her so badly. Joon-Suk tells her not to confront Kang-Hoon. At the cafeteria, Kang-Hoon ate lunch alone. Joon-Suk asks Kang-Hoon to treat Ji-Hye well. Kang-Hoon asks him if he likes her.

"Brain" will take over the 2nd and 3rd KBS2 timeframes previously held by "Poseidon" and will be replaced by "Dream High Season 2" on January 30, 2012.
On September 20, 2011 Lee Sang-Yun, Yoon Seung-Ah, Lee Hyeon-Jin and Kim Soo-Hyun were eliminated from the drama series. The four actors had their first script reading session on September 17, 2011. Staff believe those actors are not suitable for their characters due to their age, acting, and personality differences.
